For those wanting to make use of your health insurance, please note all labs will be performed at your nearest QUEST laboratory location. Please schedule your QUEST appointment in advance as they tend to fill up. Note that you must first see the doctor  before getting tested at QUEST.

COVID-19: PLEASE READ CAREFULLY!!

Keep in mind that different COVID vaccines require different time intervals in order to complete the series. The exception is the Janssen vaccine, which is a single dose.

You may want to plan ahead to obtain ALL the required shots in a spaced-out manner. For example, it is not unusual for an adult applicant to have to comply with the need to fulfill the requirements of the following vaccines: TDaP, Flu, COVID-19, and Pneumonia. This may represent a vaccine load that many people would prefer to space out over time.

If you have a significant medical condition that would exempt you from any getting any of the required vaccinations, you would need to provide appropriate medical history or a written excuse from a medical professional.

We feel obliged to make this disclaimer as we frequently encounter applicants having to meet the requirements for multiple vaccinations at the time of their medical exam. Understand that is not up to the Civil Surgeon to dismiss or withhold any of these vaccination requirements unless there is a valid medical contraindication or you choose to file a Waiver for Vaccination-Related Inadmissibility (Form I-601) based on moral or religious grounds.

HUMAN IMMUNODEFICIENCY VIRUS (HIV) UPDATE AS OF FEBRUARY 26,2024

If you have Human Immonudefiency Virus (HIV) you must complete mandatory sputum  culture samples for tuberculosis. These cultures take  6 to 8 weeks to complete. I-693 Medical Examination cannot be completed without the cultures.

Questions & Answers:
Is the COVID vaccine required? Effective July 25, 2022, any applicant 6 months through 4 years of age and older subject to the immigration medical examination must complete the COVID-19 vaccine series and provide documentation of vaccination to the civil surgeon in person.


I just received a "Deficiency Letter" requesting the medical exam but it says "DO NOT MAIL ANY DOCUMENTS AT THIS TIME"  This letter is giving you the "heads up" about the medical exam needed to be submitted to USCIS. Since the medical report in sealed envelope is only valid for 60 days (roughly 2 months) it is advised to wait until receiving the next letter/correspondence with instructions and date of submission (Interview Notice or "Request For Evidence letter")

How long is the Medical Report Form I-693 in a sealed envelope valid for? Once your medical report is completed, the sealed envelope is good for 60 days. In other words, a USCIS officer must process your medical report in less than 60 days upon the date of completion of your medical exam.

Can I update the sealed envelope after 60 days without having to redo labs all over again? Yes. We can provide you with another sealed envelope for a fee of $60 as long as your labs are not older than 6 months. Please note that even though USCIS can retain your medical exam for up to 2 years, they can still ask you to bring another (updated) medical exam to your USCIS interview even though you have previously submitted it.

Can I use my own vaccination records from my home country?  Yes. We can certify your home country's vaccination record. We understand vaccination records in Russian, French, Portuguese, Spanish, Italian, Rumanian, German, etc. If you have a vaccination record in another language please contact us at 4074805515 so that we can provide you with the best estimated medical fee.

Do I need to fast before doing the labs? No, fasting is not required. You may eat your meals (breakfast, lunch, snacks, and drinks) as normal.

Is a drug test required for USCIS Form I-693? No, drug testing is NOT required.
 
Do my labs/laboratory tests expire? Yes. Your labs cannot be older than 6 months. As they are about infectious/contagious diseases, the date of these tests must not be over 6 months.

What kind of tests are required for the USCIS Medical Exam? Children age 2-17 must do a blood test for tuberculosis. Applicants between the ages of 18 to 24 must do syphilis, gonorrhea, and tuberculosis tests. Applicants between the ages of 25 - 44 must do syphilis and tuberculosis tests. Applicants age of 45 and over must do tuberculosis test.

If I don't have my vaccination/shot record do I have to do all shots all over again? No. We run blood titers for all adults who do not possess any vaccination records on hand. Blood titer tests are harmless and give an accurate reading of your level of antibody presence for varicella, measles, mumps, rubella.

What kind of vaccines/shots/immunizations are needed? Immunizations are age-specific. Please click on "Required Immunizations" to view the list.

When is the flu season? October 1st through March 31st of each year.

Can I use my insurance to pay the USCIS medical exam Form I-693? All major health insurance policies pay for the flu and tetanus (Tdap) shots at any Walgreens, CVS, Walmart, Target pharmacy. We do use some insurance plans to pay for labs. Please call us to inquire at 4074805515.

When is the right time to submit my medical report Form I-693 to USCIS? After filing your initial application, USCIS will send you letters or notices with instructions about how and when to submit your Form I-693 Report of Medical Examination and Vaccination Records.

How fast can I obtain my medical report after my first appointment? The I-693 Medical Report is ready 7 days after your first appointment. 

Do I have to collect the medical report in person? or can you mail it to me? You can collect the medical report in person or have it mailed to you via FedEx for an additional cost. Please note, we do not mail the medical report to USCIS directly.

Do I need to pre-fill the I-693 Form? No. We prepare the I-693 Form from beginning to end. We will, however, have you double-check the information before signing it. 

Do I need to do a chest X-ray for the medical exam for USCIS? No. Only those patients who test positive on the tuberculosis blood test must do the X-ray. The chest X-ray is the second step for ruling out tuberculosis. The price of the X-ray is about $27

Do I need to do the tuberculosis test again even though I have tested positive on the PPD (tuberculosis skin test) in the past? Yes, the PPD/Skin test is no longer required. Instead, the QUANTIFERON TB GOLD PLUS/tuberculosis blood test will be performed to detect tuberculosis.
 
Do I get to know my lab results even though the medical report is in a sealed envelope? Yes, the purpose of the sealed envelope is for it to be "tamper proof". A sealed envelope means "the patient is medically clear and meets all medical requirements for USCIS". All persons with positive syphilis, gonorrhea, or active tuberculosis must get medical treatment before we can seal the medical report. 

My primary doctor just tested me for syphilis and gonorrhea. Can I use these lab results for my medical exam for USCIS? Unfortunately, the USCIS doctor performing the medical exam must be the one ordering and treating physician for all required STDs (sexually transmittable disease).

Can my primary care physician do some of my immunizations and labs needed for the USCIS immigration medical exam? Yes, you may ask to have the QUANTIFERON TB GOLD PLUS or T-Spot test done for Tuberculosis. You may also obtain all your immunizations and blood titers from your primary doctor.

We work with Blue Cross Blue Shield PPO, Florida Blue  PPO, Oscar Silver, AMBETTER.
If you have a different insurance plan, you can obtain all required vaccines and tuberculosis lab tests from your primary care physician and use it to complete your medical exam at our clinic. Please call us to obtain more information. Keep in mind that all labs for sexually transmittable diseases must be performed by the USCIS Civil Surgeon.

Also, please note, that all lab tests and some of the required vaccines have limited validity. The validity time of each test and vaccine varies on the type of lab test and vaccine type.
